Many underserved, immigrant, and low-income families in the United States face significant barriers to accessing healthcare services. Although public and private insurance options exist, navigating the healthcare system can be complex and overwhelming—particularly for individuals with limited English proficiency, unfamiliarity with U.S. systems, or limited financial literacy. Beneficiaries often experience:
•Lack of understanding of health insurance options (Medicaid, ACA Marketplace plans, CHIP, employer-based coverage)
•Missed enrollment deadlines due to insufficient guidance
•Confusion about eligibility requirements and documentation
•Limited knowledge of how to select primary care providers
•Difficulty scheduling appointments or understanding referrals
•Delayed medical care due to fear of costs
•Lack of awareness about life insurance and long-term financial protection options
As a result, many families remain uninsured or underinsured. They may avoid preventive care, rely on emergency services for primary health needs, or delay treatment until conditions worsen. This leads to higher healthcare costs, poorer health outcomes, and financial vulnerability. Additionally, without access to community life insurance education and enrollment support, families remain financially exposed to unexpected loss, further increasing economic instability.
Without structured navigation support, systemic barriers continue to prevent equitable access to healthcare services.
Purpose of the Project
The Health Access Navigation Program aims to bridge the gap between healthcare systems and underserved families by providing culturally responsive guidance, insurance enrollment assistance, and provider connection support. The initiative seeks to improve health coverage rates, enhance access to preventive care, and strengthen financial protection for families.
Key Project Activities
To achieve its objectives, the project will implement the following activities:
•One-on-one health insurance enrollment assistance
•Community workshops explaining public and private insurance options
•Guidance on documentation and eligibility requirements
•Assistance connecting families to primary care clinics and providers
•Appointment scheduling and referral navigation support
•Education sessions on preventive healthcare utilization
•Community life insurance awareness and enrollment workshops
•Multilingual health literacy materials distribution
•Ongoing follow-up and case management support
•Monitoring and evaluation of enrollment and access outcomes
These activities will provide both practical navigation assistance and long-term education support.
Project Outcomes & Beneficiary Impact
Through this initiative, beneficiaries will experience:
•Increased enrollment in appropriate health insurance programs
•Improved access to primary care and preventive services
•Reduced reliance on emergency room care
•Earlier detection and management of chronic conditions
•Improved health literacy and system navigation confidence
•Enhanced financial protection through life insurance awareness
•Reduced medical debt and financial stress
•Stronger overall community health stability
Ultimately, the Health Access Navigation Program will empower families to move from healthcare uncertainty to informed participation. By removing systemic barriers and providing structured guidance, the project will improve health equity, strengthen family financial security, and promote long-term well-being in underserved communities across the United States.
